Is there a difference between durable medical poa form and just a medical poa form?

i'm trying to be the medical poa for my mom. does it make a difference what kind of form it is for me to be able to make all decisions whether she's conscious or not? my dad is 87, so..., and mom is legally blind and has numerous med probs and doesn't want to extend her lifetime. so it's up to me to see that her wishes are done whether it's what the rest of her children want or not as i am her main med caregiver and have moved in with them to care for them. so i won't need any worse hles if worst comes to worst and i have to make a hard choice.
